The forms are in a special box on the third floor. It is very important for us to have your own point of view. Submitting an observation / proposal will in no way affect the quality of care you receive during your stay or your next hospitalization.
All written observations of patients, attendants and visitors are managed with complete confidentiality and responsibility.
